Payment of co-insurance, co-payments, deductibles, or non-covered services (when applicable) is required at the time of your visit. Verification of your plan benefits and coverage is required before your appointment. If required by your insurance or vision plan, please obtain the necessary referrals or authorizations from your primary care doctor prior to your appointment. For your convenience, we will be happy to accept your personal check, cash, Visa, MasterCard, Discover or American Express.
